This summit brings together policymakers, home care and hospital administrators, clinical and operational leaders, quality management professionals, and patient experience specialists to discuss the most pressing issues and promising strategies for cross sector collaboration and enhanced patient care and outcomes. We are pleased to be able to bring these important stakeholders together to build relationships and collectively discuss innovative ways to meet the health needs of all New Yorkers. 

9:15 – 9:45 AM     State Officials Perspectives


                              Brett Freidman, Deputy Commissioner and Acting Medicaid Director, New

In this session, you will hear perspectives from state officials on the vital benefit of collaboration among health system partners. These partners include hospitals, home health providers and physicians, as well as managed care plans, behavioral health, aging network services and community-based organizations. 


9:45 – 10:45 AM    Vision for Collaboration Across All Partners — Lessons from the

                               Hubs

This panel will explore the experiences of the vaccination hubs and the potential of this foundation for collaborative approaches in areas beyond vaccination. Panelist will answer questions about the major challenges in bringing these groups together, their differences in approaches for rural vs. urban areas, finding best practices, and what surprised them most about this collaborative work.

10:45 – 11:30 AM  Leveraging Aging Network in Collaboration

                               Becky Preve, Executive Director, Association for Aging in NYS

Hospitals, home care agencies, and county offices for aging are creatively and successfully collaborating across the state to ensure quality services and reduce system gaps. In this session, representatives from each sector will discuss how their collaborations facilitate effective care transitions, with timely and safe follow-up care, that help to avoid preventable hospitalizations, re-hospitalizations, and emergency episodes. Through their efficient use and coordination of resources these providers have overcome system and service gaps, resulting in cost-effective practices that deliver improved outcomes. 


11:30 – 12:15 PM  Health-Mental Health Collaborative


                              Catholic Health System of Long Island

                              Management and Managed Care, New York State Office of Mental Health


Co-occurring physical and mental/behavioral health conditions are among the most complex and challenging in the health care system. Individuals with these conditions are among those most frequently facing recurrent inpatient and emergency episodes, gaps in the service delivery system, impact from social determinants of health, and fragmentation in care management. This session features the hospital, physician, home care, and behavioral health sectors working together as collaborative partners under Catholic Health System of Long Island to integrate medical and mental/behavioral health services for individuals with these interdisciplinary needs. It offers a prototype of how to transform service delivery for this intensely needy and complex population, to the benefit of the individuals, their community, and the health system.
